How to Get Started
We keep it simple. Follow these steps to begin your child's journey with us—from first contact to service start. We'll guide you every step of the way.
- Step 1
Initial Contact & Paperwork
Contact the location you are interested in or visit their page on this website and complete the intake paperwork.
- Step 2
Submit Required Documents
Submit your child's Diagnostic Report, Insurance Card(s), Letter of Medical Necessity, and Referral (via email or attached to the intake form).
- Step 3
Initial Assessment
We obtain necessary insurance approvals. A BCBA conducts an assessment and skills evaluation.
- Step 4
Evaluation & Treatment Plan
The BCBA develops a personalized Plan of Care and establishes initial goals tailored to your child.
- Step 5
Insurance Authorization
The Individualized Treatment Plan is submitted to your insurance provider(s) for authorization. Staff review services and address any questions.
- Step 6
Service Initiation
Once approval is received, we review recommended hours with you and begin services.
